Explanation of * items:
1. Compensation to be divided into ten equal monthly installments, beginning upon detection of fetal heartbeat and monthly (the first of each month) thereafter. Compensation to be divided into ten equal monthly installments, beginning upon detection of fetal heartbeat and monthly thereafter. Continued payments will begin on the first of the following month and every month thereafter. Experienced surrogates may request $40,000 to $45,000.
2. Multiples fee ($5,000; each additional fetus $5,000).
3. Maternity clothing allowance is payable upon reaching 12 weeks gestation. Additional $500 for multiples due at the beginning of 20 weeks gestation.
4. Maternity expense allowance is a non-accountable monthly expense beginning from the signing of contract to 2 months after delivery. In the event of multiples, the allowance is $400.
5. ALL expenses related to travel for the medical/psychological screening, cycling, transfer, pre-natal, delivery appointments are to be covered by the Intended Parents in whole. This includes ALL mileage (in excess of 50 miles round-trip) , lodging, meals, and childcare expenses.
6. A Hold Fee is paid to the surrogate if she is asked to wait for donor choosing, matching, cycling, etc. and is paid on a monthly basis prior to signing of the contract.
7. Mock cycle with medications, and cancelled or dropped cycles (due to no fault of the surrogate) are reimbursed at $250 per cycle.
10. Transfers and Inseminations (whether IUI or HI and per month) are both $750 per cycle.
11. Invasive procedures (per procedure and per sac) are to include amniocentesis, cerclage, CVS, DNC, HSG, selective reduction, hysteroscopy, termination. Termination up to 12 weeks gestation is $1,000.00; termination 12w1d-16w gestation is $2,500.00; 16w1d + gestation is $5,000.00. Fees to be paid within 5 business days of notification of procedure.
12. Cesarean sections are reimbursed at $5,000. This is aside from any bed rest or lost wages.
13. Loss of reproductive organs fee to be paid within 5 business days of notification of procedure.
14. Housekeeping allowance is a $75 weekly allowance, beginning on the 1st day of the 20th week of pregnancy and ending 4 weeks after delivery (vaginal; 8 weeks for cesarean section) of a child.
15. Life insurance in the face value policy of $350,000 (300,000 directed toward the Surrogate's family and 50,000 directed toward the Intended Parents).
16. Premiums for medical coverage will be approximately $400-500 per month, if it is not covered by the GC, her husband, or an employer. The payments for this insurance will be the responsibility of the Intended Parents.
17. Policy in the amount of $300,000.00 as the GC’s family as beneficiary and $100,000.00 with the IPs as beneficiaries..
Any and all expenses in regard to relocation will be covered in whole by the Intended Parents.
Overnight childcare is an additional $50.00 per night per child, and not included in the Rate Per Week.
If a surrogate pumps breast milk they are compensated at the rate of $250 per week. In addition, the IP’s will need to purchase a hospital-grade breast pump, supplies, and cover all shipping cost related to getting the breast milk to them.
PLEASE NOTE:
Hospital/bed rest/lost wages in the event of doctor prescribed bed rest paid 100% as incurred less disability (if available). In the event that surrogate must take off work to complete ANY appointments due to the surrogacy, her lost wages will be reimbursed by Intended Parents. Lost wages for these appointments will be paid in full, and not covered by disability and/or sick leave.
Lost wages paid for screening/matching (surrogate and companion), transfer (surrogate and companion), delivery/birth (3 days only for companion) and any required legal proceedings (surrogate and husband/partner IF required).
Ob-gyn and hospital/delivery costs vary by hospital/doctor (most common is the 80/20 plan) and are the responsibility of the Intended Parent.
